There’s a lot of buzz about menopause and perimenopause these days. It feels like the proverbial genie has finally been let out of the bottle – but it’s a good change! It’s a watershed moment. A topic once shrouded in silence and individual suffering is now the center of new research, open communication, and widespread education.
But with the flood of information—and the time it takes for research to translate into practical advice—it can be hard to figure out what’s relevant to you. If you’re wondering, Where do I even start?, you’re not alone. Navigating menopause is challenging, and for those of us who’ve been there—dealing with night sweats, sleep struggles, or the emotional rollercoaster—it’s easy to get lost in rabbit holes of advice from well-meaning online communities.
At the Menopause Redefined Summit, Dr. Monica Christmas, Director of the Menopause Program at the University of Chicago, shed light on several topics, including hormonal changes, their impact on stress and emotional regulation, and HRT. Two key actionable themes stood out that can empower you to take control of your journey through menopause:
Preparing For Doctor Visits
Navigating healthcare effectively during menopause starts with making the most of your doctor appointments. Here’s how:
- Book the right appointment type: If you want to discuss symptoms, request a menopause consult instead of an annual exam. A focused appointment gives you and your doctor the time to address your concerns properly.
- Come prepared: Write down your questions, bring a list of medications, supplements, and alternative treatments, and have a clear idea of what you’d like to discuss.
- Choose your support wisely: If you bring someone along, make sure their presence will help—not hinder—your ability to communicate openly.
- Build a relationship with your provider: Establishing trust and continuity with your healthcare professional ensures proactive care and avoids the stress of last-minute urgent visits
The Importance Of Sleep
Sleep is a cornerstone of health, yet it often becomes elusive during menopause. Decreasing estrogen levels can disrupt the body’s cortisol cycle, stacking the odds against restful nights.
If you’re struggling with sleep, don’t add to your stress by worrying. Thoughts like Why can’t I sleep? or How will I get through tomorrow? only create a loop of anxiety. Instead, approach sleeplessness with self-compassion.
Here are a few things to try out during sleepless nights:
- Practice mindfulness techniques like deep breathing or a body scan meditation.
- If that seems too cumbersome, try out the 3-Minute Breathing Space meditation.
- Bring gratitude to mind—list 10 things you’re thankful for, either mentally or in a journal.
- If you can’t fall back asleep, get up and do something soothing: sip herbal tea, read, or take a relaxing bath.
When sleep challenges persist, consider working with a health coach who can help you develop personalized strategies for improving sleep hygiene and overall wellness.

Bridging Research And Real-Life Needs
Menopause research is growing, but it doesn’t always address the practical questions patients and clinicians face. That’s why the James Lind Alliance (JLA) has created the Menopause Priority Setting Partnership (MAPS). It aims to bridge the gap and in October 2024 identified the top 10 questions that matter most to those experiencing and treating menopause.
Some of these include:
- What are the safest and most effective non-hormonal treatments for menopause?
- How can lifestyle changes like diet and exercise support menopause management?
- Why does menopause impact cognitive function (e.g., brain fog), and how can it be managed?
- What are the best ways to address menopause-related sleep problems?
These priorities ensure that research focuses on and is funded in line with the needs of patients and clinicians, delivering insights that translate into real-world benefits.

The Importance of Managing Stress
The Menopause Redefined Summit also featured insights from Dr. Felice Gersh, a Board-Certified Obstetrician/Gynecologist and Integrative Medical Doctor, known for her book Menopause: 50 Things You Need to Know. Dr. Gersh’s perspectives on the role of hormones, particularly estrogen, shed light on how hormonal changes during menopause impact overall health and stress management.
Dr. Gersh emphasized that estrogen, often thought of as a sex hormone, plays a much broader role in the body, regulating numerous organ systems and contributing to metabolic balance. She referred to estrogen as the “life hormone,” explaining that its decline during menopause can lead to disruptions in the brain, gut, and other systems, akin to being in a state of “jet lag.”
The plummeting levels of estrogen profoundly impact the stress response, making stress management a necessity for women in midlife, not just a good idea. Dr. Gersh explained how the loss of estrogen disrupts the autonomic nervous system, which governs involuntary bodily functions like digestion, sleep, and heart rate. This imbalance can shift the body into a chronic stress state, elevating cortisol levels and leading to issues such as insulin resistance, weight gain, gut health problems, and poor sleep. These changes further exacerbate emotional challenges, increasing the risk of anxiety and depression, even in women with no prior history of such conditions.
To help women navigate these challenges, stress management becomes a critical component of midlife wellness. Practical tools and strategies, like those outlined in my previous article, Feeling Overwhelmed or Anxious? Why Changing Our Relationship with Stress Can Change Everything, offer actionable ways to reframe stress, enhance resilience, and regain balance. Exploring evidence-based practices, such as mindfulness techniques, reframing exercises, and self-compassion, can provide meaningful relief and empower women to thrive during this transformative time.
Dr. Gersh is an advocate of bio-identical hormone therapy (BHT) to address these changes, particularly during perimenopause and menopause. She likened hormone supplementation to managing thyroid disorders, advocating for early intervention to restore balance. Additionally, she emphasized the importance of progesterone, often overlooked, for its complementary role to estrogen in supporting overall health.
While Dr. Gersh’s integrative approach to hormone therapy remains a topic of ongoing discussion, her emphasis on the interconnectedness of hormones, stress, and overall health underscores the importance of a holistic approach to midlife wellness. Her YouTube channel is @FeliceGershMD.
